Create and manage AI SOAP notes

AI SOAP notes help you document client interactions by transcribing your session and generating a draft SOAP note. You can then edit the note as needed before copying it to your electronic medical health record (EMR) system.

Before you start

SOAP note recordings require access to a microphone. This could be your device's built-in microphone or an external microphone.

Ensure you allow your browser to access your microphone if prompted. The first time you go to record, a pop-up similar to the following might appear. 

For a seamless recording experience in future appointments, you may wish to select 'Allow while visiting the site' or a similar option to avoid seeing the pop-up every time.

Create a SOAP note

1. Navigate to an individual profile in VALD Hub.
2. From the Actions dropdown in the top-right corner, select  New AI SOAP note

Alternatively, use the  New AI SOAP note button at the top of the Timeline view on a profile.

From there, the recording screen will be displayed.

After ensuring the correct microphone is selected from the dropdown, it is recommended to check that the bars in the middle of the screen are moving and detecting your voice.

3. Press Start recording.
4. Obtain the patient's verbal consent, either using the consent script or in your own words.
5. Proceed with the session as normal. At any point, you may:
   • Press the Transcript button to check what is being captured in real time.
   • Press the Free text button to type in additional text notes.
6. Press Finish, and your note will begin generating.
   You can exit this screen and come back to it later. Once the note is available, its status will change from Generating to Ready.

Edit and save a SOAP note

When using the AI SOAP note feature, best practice is to review your note and confirm that the content is accurate before transferring to your EMR system.

Edit your note

Click anywhere in the note to add or delete text. You can also customize text formatting by selecting a segment and choosing an option from the menu.

Use the arrows to undo or redo any changes to your note.

Save your note

Changes are not saved automatically. When a note contains edits, the bottom toolbar shows Unsaved changes. Select Save to keep your modifications and update this status.

Add to your note

Press the  Resume recording button to discard the current note and return to the recording screen.

Keep in mind that this action will fully delete the existing note and cannot be reversed. 

You can then continue recording and regenerate a new note based on the updated content.

Manage SOAP notes

Generated SOAP notes are available through the profile Timeline view. Clicking the three dots icon next to a note's entry in the timeline gives you some additional options.

Copy a note

Press one of the Copy buttons to copy the note's contents to your clipboard – either with or without formatting. You can then paste this information into your EMR system.

Delete a note

The option to Delete note is available in the three dots menu. This permanently erases the note and cannot be undone.
