Practitioners can view and assign Outcome Measures and all corresponding information under each individual patient profile.
To view assigned Outcome Measures:
- Login to VALD Hub.
- Navigate to a Profile.
- Select the Outcome Measures tab.
Assigning an Outcome Measure survey:
Once you are on the individual patient profile and under the Outcome Measures tab, you will have the option to a 'Assign Outcome Measure':
- Click on Assign Outcome Measure.
- Select the Outcome Measure to assign by searching or filtering.
- Select Confirm.
Once you click confirm it will present you with a new screen to confirm the parameters for the Outcome Measure.
NOTE: Side will only appear on relevant/appropriate Outcome Measures.
- Select Side
- Select Start Date
- Select Number of Surveys in series
- Select Rate of Recurrence
- Select Assign Outcome Meausure
